The subacromial space is a tiny gap, usually only about 1.0 to 1.5 centimeters high. When that little cushion gets inflamed, it swells up and takes up way too much room in an already tight space, leading to a vicious cycle of pressure and pain.

Shoulder bursitis occurs when the bursa, a tiny fluid-filled sac that acts as a shock absorber under the bony tip of your shoulder, becomes inflamed. This little grease packet is designed to prevent bones and tendons from rubbing together. However, repetitive overhead movements or a sudden fall can cause the bursa to swell like a soaked sponge. This inflammation takes up too much room in the shoulder joint, leading to a deep, nagging ache that can disrupt sleep and daily activities.
It is actually rare for the bursa to be inflamed entirely on its own. Instead, shoulder bursitis typically goes hand-in-hand with other conditions like rotator cuff issues or impingement syndrome. In fact, these related conditions can represent up to 66% of all reported shoulder pain. When the bursa swells, it gets pinched during movement, often as part of a team effort of inflammation involving the surrounding tendons and the tight spaces within the shoulder anatomy.
The primary symptom of acute shoulder bursitis is a persistent, deep ache in the shoulder that often feels like a toothache in the arm. Many people find that the pain intensifies at night, making it difficult to roll over or find a comfortable sleeping position. Beyond sleep disruption, it can make simple tasks like washing your hair or playing sports like tennis a significant struggle. This pain is a signal that the inflamed bursa is being pinched within the shoulder joint.
