They worried about two equally problematic outcomes: if Khan died after receiving the drug, locals might believe white foreigners had killed him, potentially triggering violence against the camp; if he survived when others couldn't be saved, it would violate their ethical principles. Despite appeals from WHO doctor Tim O'Dempsey, who had worked with Khan in Kenema and advocated giving Khan the choice about taking ZMapp, the camp staff unanimously decided against administering the drug. A plan emerged to evacuate Khan to Europe with O'Dempsey administering ZMapp during the flight, but no country would accept an Ebola patient. As Khan's condition deteriorated, Michael Gbakie forced his way into the red zone to see him after getting permission from Sierra Leone's health minister. Finding Khan in filthy, soiled clothing, Michael cleaned and changed him. Khan asked for a soft drink before requesting rest. Later, Mohamed Yillah, despite his own Ebola infection, somehow summoned strength to carry Khan outside for fresh air. When Michael returned, he found Khan gasping for breath by the fence. Khan managed to say "Michael..." before his breathing stopped. The revelation that Khan was denied ZMapp - a drug that would soon save American patients - caused international outrage. Camp manager Anja Wolz later admitted through tears, "Now, knowing that ZMapp works, it was probably the wrong decision."

The Miracle Medicine: ZMapp's First Human Trial

While Khan fought for his life in Sierra Leone, American doctor Kent Brantly was treating Ebola patients at ELWA Hospital in Monrovia, Liberia, when he developed symptoms himself. His condition deteriorated rapidly - hemorrhaging severely with black blood in his diarrhea, bright red eyes, and the classic Ebola rash spreading across his chest and torso. His fever spiked to 104.5F, and he became too weak to get out of bed. Meanwhile, Lance Plyler, emergency operations director for Samaritan's Purse, discovered that a course of ZMapp was available in Sierra Leone. Arrangements were made to retrieve it via bush pilot or UN helicopter, with Lisa Hensley agreeing to join the mission despite her supervisor's objections. After a harrowing helicopter flight through thunderstorms over jungle-covered mountains, Hensley and Marine Lieutenant Colonel Bryan Wilson retrieved the ZMapp and delivered it to Lance Plyler at ELWA Hospital. Plyler now faced the agonizing decision of which patient should receive the single course of the experimental drug - Brantly or Nancy Writebol, another American missionary who had also contracted Ebola. Despite being critically ill with rash covering his body, shallow breathing, and hemorrhaging, Kent selflessly told Plyler to "Give the drug to Nancy." However, as Brantly's condition worsened to the point of death, Plyler decided to split the doses between both patients. With Kent Brantly actively dying, Plyler raced against time to administer the experimental ZMapp. Finding Kent's bottle frozen solid, he retrieved Nancy Writebol's partially thawed dose. Just minutes after the infusion began at 8:00 p.m., Kent started shaking violently - either rigors from the antibodies attacking the virus or possibly death tremors. Remarkably, his temperature plummeted from 105 to 100 degrees within fifteen minutes. After thirty minutes, the shaking stopped, and Kent sat up. By 9:00 p.m., he was able to walk to the bathroom with assistance, despite only 12% of the dose having entered his bloodstream. The drug had destroyed Ebola in Brantly's body with astonishing speed, revealing the potential of biomedical defenses against emerging viruses. Nancy Writebol received her dose via bone marrow infusion, experiencing itching but no dramatic improvement, though she survived. Both were evacuated to Emory University Hospital in Atlanta where they made full recoveries. The remaining ZMapp courses were used to treat patients worldwide with mixed results.

Lessons for Tomorrow: Preparing for the Next Viral Threat

The 2014 Ebola outbreak eventually claimed over 11,000 lives across West Africa before being contained. But beyond the immediate tragedy, it revealed critical vulnerabilities in our global health systems and offered important lessons for future outbreaks. One of the most shocking revelations came from genome sequencing: the hospital staff at Kenema caught Ebola from each other, not from patients. The chain began when ambulance driver Sahr Nyokor visited friends without protection and contracted the virus. After cutting his scalp, Nurse Lucy May washed his blood - a simple act of care that infected her. When Lucy died giving birth to a stillborn child, the virus spread to her caregivers: Auntie Mbalu Fonnie, Princess Gborie, Sia Mabay, Fatima Kamara, and Alex Moigboi all caught the virus while trying to save Lucy. Dr. Humarr Khan too died from Lucy May's virus, proving he didn't fail his people but died with them. This pattern reveals the crucial importance of proper protective equipment and protocols, even when resources are scarce. It also highlights the particular vulnerability of healthcare workers during outbreaks - they formed what Preston calls "a thin, dissolving line of sacrifice between the virus and the rest of humanity." The outbreak also demonstrated how quickly a virus can evolve to become more dangerous. The A82V Makona Variant that emerged during the outbreak was four times better at infecting human cells than the original wild Ebola. This mutation involved just one letter change in its 18,959-letter genetic code, altering a single amino acid that allowed the virus to better attach to receptors on human cells. This tiny change essentially created a "humanized Ebola" that spread more efficiently among people. While Ebola was eventually contained, virologists warn that other pathogens pose even greater threats. The morbilliviruses, particularly Nipah, represent a concerning threat - a Level 4 virus that affects lungs and brain, causing personality changes and brain liquefaction. Though not currently highly contagious, Nipah and similar viruses could evolve greater transmissibility. If a brain-destroying virus spread like flu, everyone's risk factor would simply be breathing. Despite progress with treatments like the Pan-Ebolavirus Cocktail and Nipah Cocktail, the U.S. has only 542 biocontainment beds nationwide - 142 for hemorrhagic fevers and 400 for airborne viruses. This raises a crucial question: if a Level 4 virus infected a million people, could hospitals handle it? Could epidemiologists trace and break transmission chains? The human species does have advantages in this fight: self-awareness, teamwork, and willingness to sacrifice. As Wahab the Visioner believed, we can change our fate if we see it coming. If viruses can change, we can change too. The Ebola crisis ultimately teaches us that emerging viruses are not just medical challenges but human ones. They test our scientific knowledge, our healthcare systems, our cultural practices, and our capacity for both selfishness and sacrifice. As Preston writes, "The past is unpredictable." The Ebola epidemic wasn't an anomaly but part of a pattern - a series of small accidents and unnoticed events that cascaded into horror. This was the shockwave of an emerging virus coming out of the ecosystem, magnifying in people before finally dying out. But what might the next shockwave be like? And will we be ready?
