What began as one man's crusade became a national phenomenon as Hawaii, Illinois, Idaho, New York, Rhode Island, Texas, and Michigan all adopted versions of the triplicate rule. Madden's filing cabinets of carbon copies had transformed into an overstory-a belief system that doctors couldn't be trusted with opioid prescribing without government oversight. Fifty years later, a second competing overstory emerged through Russell Portenoy, a brilliant and charismatic doctor who believed pain itself should be treated as the primary concern. Portenoy became evangelical about opioids, calling them "a gift from nature" that "can be used for a long time, with few side effects" where "addiction and abuse are not a problem." By the mid-1990s, Portenoy's ideas gained traction, and the number of triplicate states dropped to just five: Texas, California, New York, Illinois, and Idaho. Purdue Pharma, owned by the Sackler family, strategically exploited this divide. After developing OxyContin-a powerful extended-release oxycodone tablet without the safety mechanism of acetaminophen-they targeted their aggressive marketing campaign at non-triplicate states like West Virginia, Tennessee, and Oklahoma, while avoiding triplicate states like New York, California, and Texas. The impact was profound and enduring. States without triplicate programs consumed opioids at dramatically higher rates-Nevada and West Virginia used nearly three times more opioids per capita than Illinois. Economist Abby Alpert estimated that if New York had Massachusetts's opioid-overdose rate from 2000 to 2019, an additional 27,000 New Yorkers would have died. At the heart of McKinsey's OxyContin marketing strategy was a remarkable data analysis that divided prescribers into deciles. Their chart revealed that while 6.17 million OxyContin prescriptions were written between January and July 2013, the distribution was dramatically uneven. The bottom decile contained 99,825 doctors who wrote just one prescription on average during this period. Meanwhile, the top decile consisted of only 358 doctors who each wrote an average of 247 prescriptions. McKinsey's advice was blunt: stop wasting resources on the "low-decile prescribers" (0-4) who made up more than half of Purdue's sales calls. Instead, focus exclusively on the "Core" and "Super Core" physicians in the top three deciles-just 2,500 doctors who were driving the epidemic. Purdue implemented this strategy aggressively, creating a point system that rewarded sales representatives for concentrating on these high-volume prescribers. Ultimately, the top 1 percent of doctors accounted for 49 percent of all opioid doses prescribed, with some prescribing 1,000 times more than the average physician. Despite the overwhelming majority of doctors treating opioids with appropriate caution, this tiny fraction of superspreaders was enough to trigger the worst overdose crisis in American history-a stark demonstration of the "Law of the Very, Very, Very Few." The reformulation of OxyContin in 2010 created a catastrophic shift in the opioid crisis. When the pill became impossible to crush and snort, addicted users switched to heroin, then to fentanyl, and finally to dangerous combinations including tranquilizers and veterinary drugs. By the early 2020s, the epidemic was claiming nearly 80,000 American lives annually-a line that should be decreasing two decades into a pandemic, not climbing. Despite Kathe Sackler's claim that she couldn't have done anything differently "knowing what I knew then," the evidence suggests that epidemics follow rules and boundaries. They're subject to overstories that we create, change at identifiable tipping points, and are driven by identifiable people. The tools to control epidemics are available-we can either let the unscrupulous use them, or pick them up ourselves to build a better world.
