According to Boyle’s Law, the ratio of pressure change is highest closest to the surface. At a depth of only 33 feet, the ambient pressure is double that of the surface, meaning a lungful of air will attempt to double in volume if a diver ascends while holding their breath. Because human lungs are not elastic like balloons, this expansion can cause the lung tissues to tear, leading to life-threatening conditions like pulmonary barotrauma or an Arterial Gas Embolism (AGE).
The primary difference lies in the cause and the timing of the symptoms. DCS, often called "the bends," is a chemistry-based issue governed by Henry’s Law, where nitrogen dissolved in the tissues forms bubbles during or after ascent; symptoms can take minutes or even hours to appear. In contrast, an AGE is a mechanical injury caused by lung overexpansion, where air is forced directly into the bloodstream and travels to the brain. Symptoms of an AGE, such as unconsciousness or paralysis, are almost always immediate upon surfacing.
Nitrogen Narcosis, often called the "martini effect," occurs because the partial pressure of nitrogen increases as a diver goes deeper, as explained by Dalton’s Law. At depths around 100 feet, this high pressure affects nerve cells, causing symptoms similar to alcohol intoxication, including impaired judgment, euphoria, or anxiety. To manage this, commercial divers often switch to specialized gas mixtures like Trimix, which replaces some nitrogen with helium to keep the diver’s mind clear at extreme depths.
The umbilical acts as a multi-functional lifeline that connects the diver to a surface support team, providing a constant supply of air pumped from the surface rather than a limited supply carried on the back. Beyond air, it contains communication wires that allow a supervisor to monitor the diver’s breathing and a pneumofathometer hose that provides exact depth readings. This system shifts the burden of safety monitoring to a surface supervisor and tender, though it requires the diver to carry a small "bail-out" cylinder for emergency use if the surface supply fails.
The most critical first step is the administration of 100% high-flow oxygen, which creates a pressure gradient that helps dissolved bubbles move out of the body. The diver should be kept in a horizontal position on their back or side; the older "head-down" position is no longer recommended as it can increase brain swelling. Finally, the diver must be transported immediately to a recompression chamber, where controlled pressure is used to shrink the bubbles and safely dissolve them back into the bloodstream.
