    五十肩の正体は、肩の関節を包んでいる「関節包(かんせつほう)」という袋のトラブルです。本来は自由に動くためにゆとりのある袋ですが、何らかの原因で炎症が起き、腫れたり厚くなったりすることで、最終的にベタッと癒着して縮んでしまいます。この袋の収縮が物理的なストッパーとなるため、肩が上がらなくなります。

    発症から約2週間から9ヶ月ほど続く「疼痛期(炎症期)」には、無理に動かすのは逆効果です。この時期は炎症が非常に強いため、無理なストレッチは火に油を注ぐようなもので、さらに症状を悪化させる恐れがあります。まずは安静を優先し、整形外科などで消炎鎮痛剤や注射を用いて炎症を抑えることが最優先となります。

    一つの目安として、自分だけでなく「他人に動かしてもらっても肩が動かないか」という点があります。五十肩は関節の袋自体が縮んで固まっているため、他人が動かそうとしても物理的な壁に当たったように止まります。一方で、石灰沈着や腱板断裂などの他の病気の場合は、自分では痛くて動かせなくても、他人に動かしてもらうと動くことが多いのが特徴です。

    糖尿病の人は五十肩を発症するリスクが高く、さらに治りにくかったり両肩とも発症したりする傾向があります。これは、高血糖状態が続くことで体内のタンパク質と糖が結びつく「糖化」が起き、関節包のコラーゲンが硬くなったり炎症が起きやすくなったりするためだと考えられています。

    放置することはおすすめできません。放置すると自然治癒までに2〜3年かかることもあり、その間に睡眠不足や他の部位への二次被害(腰痛など)を招く恐れがあります。また、約10〜20%の人は数年経っても肩の硬さや痛みが残ってしまうという報告もあり、一生可動域が制限されるリスクを避けるためにも早期の適切な治療とリハビリが推奨されます。
