By 1917, London has changed dramatically. Lilith volunteers at St. Mary's Convent in Holborn, distributing food to starving Londoners affected by wartime shortages. Though engaged to Louis, who nearly died trying to save Freddie, Lilith continues postponing their wedding, using the war as an excuse. She still loves Bram but let him go, fearing she couldn't protect him after failing Freddie.
At night, unable to sleep, Lilith communes with spirits in the garden. A disturbing encounter with the ghost of a young soldier named Alfie James, horrifically wounded in battle, leaves her shaken when he and other spirits demand she use the Elixir to resurrect them. Lilith fears she's unleashed a terrible hunger among the spirits, who are no longer content to stay in the Land of the Dead, especially as war swells their numbers.
Meanwhile, Stricklend delivers a brutal ultimatum to the earl of Winchester: either obtain the Elixir and Great Secret, or they will kill his son Louis. When the earl protests that only Lilith knows the Great Secret, Stricklend coldly suggests bringing Lilith herself to them. The earl eventually betrays Lilith, dragging her from the chamber as he clutches the Elixir casket. Before they can escape, a German bomb - the largest ever dropped on London - finds its mark on Number One Fitzroy Square.
By 1919, Lilith has survived the bombing and established a new life in a penthouse apartment overlooking the Thames. For the coven's new sacred space, she created a glass-domed observatory on her roof garden instead of rebuilding underground. She's engaged to Louis, now an earl since his father's death in the bombing, though she still harbors feelings for Bram.
Bram, meanwhile, has returned from fighting in Africa with a rifle wound in his shoulder. Despite his father's disapproval, he's maintained his artistic pursuits alongside working at the family steelworks. When he learns of Lilith's upcoming marriage, he resolves to make one final attempt to win her, believing there is "no greater magic than love."
At Bram's exhibition in the Dauntless Gallery, he publicly declares his loyalty to Mangan who taught him "to have the courage to be honest in my work." The tense silence is broken by a single person clapping - Lilith, who has come despite her engagement. In the rose gardens of Regent's Park, Lilith breaks Louis's heart by returning his engagement ring, explaining she cannot go through with their wedding. Though he tries to hide his pain, Louis's sadness is palpable as he accepts that Lilith truly loves Bram.
The final confrontation occurs when Nicholas Stricklend arrives uninvited at Lilith's roof garden. He demands the Elixir, threatening Bram by suspending him magically over the edge of the building. As Stricklend prepares a deadly spell, Lilith steps between them, hurling her own half-formed magic. His spell strikes her instead of Bram. As she lies dying in Bram's arms, Lilith reveals the Great Secret - the final ingredient for the Lazarus Elixir: three drops of blood from a newborn baby, collected within its first hour of life.
Refusing to accept her death, Bram races to St. Mary's Convent to retrieve the Elixir box, then convinces Louis to help resurrect Lilith. Meanwhile, Stricklend attempts to use the stolen secret to resurrect Perry, but the corrupted magic fails, causing Perry's body to rot rapidly before demons drag him into the abyss.
Lilith awakens with heightened senses and powers, her body crackling with magic. At the cemetery, she confronts Stricklend beneath a mighty cedar tree. When he threatens Louis and Bram, something ancient awakens within her - the full power of a risen witch. Her feet leave the ground, her skin glows luminous, and with a single, pure note of sound, she topples headstones and sets the graveyard ablaze. She banishes Willoughby to the Darkness with an ancient spell, then transforms ivy into a Silver Thread that she uses to whip Stricklend across the face before sending him plunging into the abyss.
As the spirits gather around Lilith, kneeling and whispering, "Welcome, Queen of the Night," she asks Bram if he can still love her despite what she's become. He simply replies, "Where else would I be?" As dawn breaks, they prepare to face the new day together - a witch who defied tradition and an artist who dared to love her, united against whatever darkness might come.
