Common symptoms include weight gain, reduced libido, insomnia, mood swings, memory problems, skin changes, recurring infections, and vaginal dryness. Hot flushes, experienced by 80% of menopausal women, can range from mild warmth to incapacitating episodes with sweating and heart palpitations. Genitourinary syndrome of menopause (GSM), previously called vulvovaginal atrophy, affects up to 50% of menopausal women. Unlike other menopausal symptoms, GSM is chronic and unlikely to improve with time. The condition causes thinning and weakening of vaginal and urethral tissues, leading to persistent urinary problems, recurring infections, vaginal dryness, inflammation, and painful sex. Despite its prevalence and significant impact on physical comfort and emotional wellbeing, approximately one-third of women with GSM never consult a doctor, believing these symptoms are simply something to "put up with." Hormone replacement therapy (HRT) has been the primary menopause treatment for over fifty years, yet remains controversial. Modern HRT options include pills, patches, gels, and vaginal tablets for GSM symptoms. Some women seeking alternatives turn to bioidentical hormones, promoted by celebrities like Oprah Winfrey and Kim Cattrall as "more natural" options derived from plant sources. However, the NHS explicitly warns against these unregulated treatments due to unclear safety and efficacy. A gradual shift toward openness about menopause is emerging, with more public figures discussing it. However, the tendency to treat menopause as merely comedic remains problematic, as Aminatou Sow notes: "It is always a punchline... And I'm like, let me tell you, this is not funny." Workplace challenges persist, with a quarter of women considering leaving their jobs due to menopausal symptoms. Breaking the silence around menopause requires comprehensive education involving not just women and doctors but employers, designers, writers, policymakers, and family members. While menopause can bring renewed energy and freedom for some women, others struggle through difficult periods. In both cases, women deserve support and representation.

Beyond the Binary

This chapter examines how genitalia relates to gender identity, particularly for transgender people who face invasive curiosity and cruelty about their bodies. While the author has advocated for openness about vaginas throughout the book, she acknowledges that this approach isn't always practical or safe for the trans community, who often face fetishization of their genitals and derisive public commentary. The chapter references Stonewall's definition of "trans" as an umbrella term for people whose gender doesn't match their birth-assigned sex. CN Lester describes the trans category as potentially "uncomfortable," filled with societal fears about disruption and change. The author emphasizes that recognizing trans rights means acknowledging that not all women have vaginas and not everyone with a vagina is a woman. Transitioning encompasses various paths, from gender-confirmation surgery to hormone replacement therapy, cosmetic procedures, or simply changing names and pronouns. Some trans women like Ani question the medical establishment's focus on making trans bodies conform to cisgender norms, noting her "arrival at womanhood was completely spiritual" rather than physical. Meanwhile, Buck Angel, a trans porn producer, chose against lower surgery, writing about learning to accept his vagina and encouraging more open conversations about trans bodies and pleasure. Personal stories can be powerful tools for change, as demonstrated when thousands of Irish women shared abortion experiences before the successful 2018 referendum to legalize abortion. Yet nobody should feel compelled to disclose intimate details about their sexual and reproductive health to gain basic human rights. Trans people face particular challenges when sharing such stories, risking hate and discrimination. The author rejects the notion that caring about vaginas and trans rights are mutually exclusive positions. When Janelle Monae released "Pynk," celebrating black women and queer sexuality with vulva-inspired costumes, some criticized it as "vagina-centric" feminism that excludes women without vaginas. This dismissal risks further stigmatizing vaginas and vulvas while ignoring that women worldwide face abuse specifically because of these body parts. Her feminism acknowledges that trans women are women and men can have vaginas, while also recognizing how cis women's bodies are used to oppress them. Education must acknowledge that neither gender nor sex is strictly binary. Up to 1.7 percent of people have intersex traits, with sex characteristics that don't neatly align with typical binary notions. Since the 1930s, doctors have performed often unnecessary surgeries on intersex babies to "normalize" their genitalia-practices increasingly recognized as human rights violations. Ultimately, the author concludes, we are all more than our genitals-more than our vaginas, ovaries, or hormonal fluctuations. As Chimamanda Ngozi Adichie writes, "Gender as it functions today is a grave injustice." We all deserve lives not defined by our genitals and gender-we must stay furious until society recognizes that.
